Being prepared to see the Specialist ensures vital medical information is provided before your initial appointment.
Vital Information Needed:
1. - Valid Referral Letter from your Medical Professional (This is vital as it provides context for your appointment from your referring Medical Professional).

3. - Signed Patient Consent - 48hrs prior to your appointment, you need to have downloaded, read, signed and sent back your Patient Consent acknowledging Service Costs and Your Consent to Proceed. Attach it or send signed consent to: contact@hormonalhealth.com.au
Bulk Billing is not offered, this Specialist Service requires up front payment at the time of service, with any applicable Medicare Rebates applied and lodged on your behalf. Our Patient Disclosure Form outlines the different billing scenarios. Consent is required before Dr Attendance.
Invoices are sent to you via email for payment, and generally it takes 48hrs for Medicare Rebates to arrive in your the bank account you have nominated with Medicare (Where applicable). The Patient Disclosure Form shows what MBS Code is likely applicable, these codes are itemised in the our system and Lodgment of Medicare is done after your upfront payment.
Our systems track payment of service invoices and history of lodgments to Medicare. As a regulated industry any amounts you may receive back from Medicare are based on the nature of your visit with the Endocrinologist.
Lodgment to Medicare occurs after your payment. If you have an outstanding invoice, you will be notified by email of the amount outstanding, the date of your attendance, with link for payment. We will also attempt to make contact with you by phone, we can process payments in person and over the phone if you struggle to pay via secure link via email. If after failed attempts of outstanding invoices not being paid, the practice reserves the right to notify credit reporting agency of non payment. If this is the case your Credit Score may be negatively impacted.
The Patient Disclosure Form, provided you with upfront clarity in relation to cost of service. All initial Consultations require the Dr to review your scenario, any background information and prior test results relevant to your care. At the end of your (expected 45min Appointment). Your signed consent of the Patient Disclosure must be received 48hrs prior to your appointment. If not received 48hrs prior your appointment, your appointment will be cancelled, with the time being offered to patients waiting to see the Doctor.
